HDPE is a high density polyethylene product that comes in various sheet sizes 
as well as blocks and rounds. I don't know about self healing but is easy to 
machine and offers very little friction. I think another good alternative would 
be phenolic. Also comes in various sizes including round stock.

The only "self-healing" stuff I have experience with is called "Silly Putty"!

What you may be referring to is UHMW Plastic.  Attached is a screen grab of a 
piece about the size you might be wanting.

Has anyone ever made lead screw blocks out of wood?

If so - what were the results?

I know they made "bearings" for

assume one of The Group members has used wood for a screw block.

I'm sure the type & quality of the species you choose will have a "bearing"
