The accepted/recommended practice is to use [--?--] when you have an unknown
surname. Many leave it blank (as I did for some years) as this indicates (by
default) that it is not known......but this can become confusing and might
leave unanswered questions. What I really dislike is obtaining data where
the name is "Roy" or perhaps "Dana" - - can be either given or surname so
must be careful.
