I think it means that the cities are independent of the counties.  Most 
cities in Virginia are this way.  They have their own governments separate 
from the county.

There are several places I've found in the Geo Location database like the
following:
Saint Louis, Saint Louis (city), Missouri, USA
Does anybody know what the "(city)" means in the county location?
