Almost. This works for me, a two step process. If I want to find the
individuals *without* an alternate name:

Step 1:
1. Search >> Detailed Search and set this:
a. Look for whom = Individual
b. Where to look = Alt. Given Names
c. How to look = Contains
d. What to look for = <blank>
Yes, this gives me a list of people with an alternate name. I am not done
yet.

Step 2:
1. In the Search list >> Options, click Advanced Tagging
a. Clear all tags to be safe
b. Click make changes on #1 (or whatever your favorite tag is)
c. Click Everyone in Search list.
d. Close, Close Search List
2. Now click Search >> Find (or just Ctrl F)
3. Search >> Detailed Search but this time set the fields to:
a. Look for whom = Individual
b. Where to look = Tag 1 (or whatever tag you chose above)
c. How to look = Equal to
d. What to look for = Untagged
Click Create list. Now I have a list of people for whom I have no AKA for.
Is that what you want?
