Fanless video cards definitely help with the noise.  I had a relatively silent 
computer that now sounds like an airplane on takeoff after I added the video 
card.

I ended up simply enclosing the whole box in a separate cabinet.  I hate 
computer noise.  As I get older, I seem to hate it even more.

I go out of my way to find and only buy video cards with no fans. I have
had to many fail, and a few fry as a result. Plus finding and replacing
fans for video cards is a PITA and they tend to be a bit more expensive
than normal fans.

Not sure a fanless video card helps with noise or heat, just helps with
system reliability IMHO. Just make sure something is moving the air
around in the box.
