Your tftp server seems not to be running.
Make sure you have a tftp server installed at all, then
check that in /etc/xinetd.d/tftp a line is saying "disabled=no" (if using
xinetd) or that tftp appears in /etc/inetd.conf.
If this doesn't help (don't forget to restart xinetd/inetd after changing
configuration), please post the result of entering
netstat -l -n -p | grep 69
