I think you are being too hard on yourself. Not one of us hasn't made a similar mistake, and after only 3 months, seriously it is not a long time. I know I am certainly not perfect.
Has your boss actually said anything to you? Or are you just being hard on yourself? If your boss taught you the procedures, maybe she just needs to go over it with you again. One thing is for sure in this business though..you really need to grow a thick skin. Things like this are going to happen, especially when you're new. It's how you handle it that determines your success. Like I said, hang in there, but ask your boss for help if you need it !
