In most of my 100s of installs, I tied the ground at the OUTPUTS and
cut at the inputs. Then I utilized a star ground with heavy gauge
wire. Quiet as a son-of-a-gun every time. Tie both ends of all mic
lines!!!!
You will occasionally find a device that is unhappy with this
arrangement and it will require some monkeying around to make it happy.
FWIW, any simple system, IE under 10-15 devices or so, connect all
grounds and you will be perfectly happy, especially if there are all
balanced or all unbalanced devices.

um yeah...just finished my 96 point patchbay, going to the console

The wonderful thing about EDAC connectors is that they are really
easy
to reconfigure if they aren't right.
I strongly reccommend that you have your cables wired with the
sheild terminated at one end only (except the Mic sources, of
course).
