It shows to use a driver for an NCR5380 SCSI controller.  The sample command
they show is 
     insmod g_NCR5380 ncr_irq=255 ncr_addr=0x280 dtc_3181e=1 
You may have to play around with the ncr_addr part.  They list a whole bunch
of IO addresses the controller card can use.
