If the original file checked in is HTML, I'm not sure the CMS Web
interface will allow you to rename it to a .mdtext file.  I'd just add
a new index.mdtext or whatever.  If the file is mdtext then the CMS
web interface will give you an mdtext-specific UI with a preview.

The CMS is a shortcut to making simple changes to HTML or Mdtext or
any other text-based files on the website.  Behind the scenes it uses
SVN to check in the files.   You can always do everything in SVN if
you want.
