Can you post the output from " fdisk -R -W - /dev/rdsk/c4d0p0"?
I'd like to see the disk dimensions (sectors/track, track/cylinder,
cylinders) and the existing  fdisk partition entries at the end.
You can leave out the list of "sysid: 1 DOSOS12 ... 239: EFI_FS".

E.g. something like this:

# fdisk -R -W - /dev/rdsk/c8t0d0p0

* /dev/rdsk/c8t0d0p0 default fdisk table
* Dimensions:
*    512 bytes/sector
*     63 sectors/track
*    255 tracks/cylinder
*   48641 cylinders
...
* Id    Act  Bhead  Bsect  Bcyl    Ehead  Esect  Ecyl    Rsect    Numsect
  191   128  0      1      1       254    63     1023    16065    781401600

You did use option "5. Exit (update disk configuration and exit)"
in fdisk, before restarting the GUI installer?
