Actually the F-104 was relatively safe in US service, as well as 
Japanese service. Of course, the US never bought all that many (Canada 
was the biggest user, by a large margin) and used them as high-altitude 
interceptors, which is what they were designed to do. That reduced the 
safety issues down to landing.

The Lightning was little better for the role the Canadians and Germans 
were doing, the Thud on the other hand...
