I think you under estimate the time and effort that is required to test for every effect a change has through the whole package now. Altium have sent hot fixes in the past with about a day, for big issues, is the ODB++ internal pad thing a big deal ? can't it be generated from CAMtastic as a work around or just leave unconnected internal pads on ? Who is offering patches within hours/days ?
