Palo Verde provides the following response:

Does your station survey all tools returned to the RCA tool room / tool crib
for contamination?
Yes

IF so...
Does your station use a tool monitor / SAM as part of the survey process?
We use a tool monitor in each of the unit tool rooms

IF so...
What set point is used for the tool monitor / SAM?
We established an alarm setpoint value of 20,000 dpm

Are surveys performed by RP Technicians or tool room craft?
RP or Tool Room Craft

IF RP Technicians...
Is this task routinely assigned or by request?
RP Tech's perform this activity as part of post job coverage activities as
tools and equipment are returned to storage

Are unsurveyed tools ever available for craft use or segregated until surveyed?
If tools are not surveyed, they are placed in a designated area until they can
be surveyed; rejected items are placed in a drum and sent to the
decontamination facility
