I had a fork crown bolt shear or loosen enough to allow my berthoud front rack 
(with loaded Wald basket) fall forward and lock the front wheel... I stopped 
instantly and went over the handlebars.  It collapsed the fork and I ended up 
breaking a small elbow bone which healed quickly.

I got the frame checked out and aligned by Chris Bishop in Baltimore, then got 
a new fork ordered from Riv (before they started selling all the forks cheaply) 
and now I'm just waiting to get it back from the powder coaters so I can 
reassemble.  

I was pretty lucky, no other bikes or pedestrians were involved and I was on a 
bike path not a busy street.  Keep an eye on your rack hardware! :)
