It hasn't happened to me for a while now but when it did it was mp3
files and using id3v2.3 tags.  It would be something like this. 
(right)02 - Let It Be  (wrong)02 -Let It Be  That would put the track
into a separate album.  Once I added the space and rescanned it would be
correct.  I can't seem to find my setting for guessing right now but
I've always relied on having proper tags.  And only Id3v2.3  The times
it happened it really bugged me as I thought the file name wouldn't
matter as long as the files were tagged right and in the same folder,
which was the case.  It took me a bit to find the problem as I didn't
even consider files names. But like I said, it's been quite a while
since this has happened so I don't know if it still has this problem.
