You should call Tony from Mopec... we got ours from him and he goes the extra 
mile...
He was there at installation and did a few separate in-service training.  He 
and his engineers even customized the CFM to give us greater suction!

We purchased Thermo's Gross Lab Senior grossing stations.  A rule of
thumb is for these to pull at 450-500 CFM.  If you purchase the hoods
that include the fan motor, these are factory set to this.  We hooked
ours in to the hospital ventilation system so it was a bit trickier to
achieve this velocity.  We currently are pulling at 550 CFM per hood.

How To Cut Nails
1. section nail and place on superfrost plus (or adhesive slide)
2. shake excess water off slide
3. place 1-3 drop formaldehyde (10% buffered ok) at the bottom of a plastic 
coplin jar
4. place cut slides in jar
5. tighten cap
6.Place on hot over 60 degrees...15 -20 minutes
7. remove (do NOT rinse) and cool
8. stain or deparifinize

The aldehyde in the formalin moves through the tissue and with the heat, it 
vaporizes... it pushes the tissue against the glass and it becomes 'set' almost 
permanently!

[Histonet] Frozen sections

Question:  When doing frozen section on a lung mass (not fatty- just regular), 
and it chatters, what is the cause?

The cryostat was set at -26, we used the normal OCT media, and cut at 
5microns.

I prefer not so cold (like -23 to -24)   and I cut sections at 4, 5 , and even 
6 to see if it would help... still chatter!The pathologist said it looked 
all necrotic-like!!

Without knowing for sure if the tissue's morphology is due to its real state 
(ie- full of cancer) or a cause of freezing artifact, I need some help from any 
histotech that is a Frozen Section Guru :)

But then again, 2 mislabeled slides does not justify termination (well, 
depending on the frequency)... I guess I would be looking at performance 
improvement.

Janice hit the nail on the head and is very right... bar-coding is the way to 
go! That was one of the big topics at this year's NSH.

I'd be happy to share mine but it is very strict.  People are given a final 
written warning for the first mislabel not caught in the department and can be 
fired for the second within a year.
I highly suggest the Ventana Vantage (there are others but in my opinion 
Vantage is the best) bar code system for patient safety.  It also relieves the 
stress on your Histo techs because the bar coding helps them assure they have 
the correct slide, block, etc.  Techs are hard to find and if we start firing 
for inevitable human error we will cause even more tech shortages.
When you look at all the costs that a mislabel can incur, (law suits, replacing 
personnel, rework, etc.) the cost of the software is not too bad.

When you place dye on any tissue, make sure you pipette some bouins on to the 
tissue... Apparently the bouins sets the stain... afterwards dab the tissue 
with paper towel to sop up the excess dye/bouins.

Some dyes are better than others, but try the bouins first

We may be encountering some issues with our tissue marking dyes 'clogging' up 
our processors.  Ive heard of a system where a coloured 'bead' is embedded 
right beside the tissue, and subsequently cut onto the slide.  This does not 
mark the margins obviously, but is used as a method to track like specimens 
that are grossed, embedded and subsequently cut in a row. (red, orange, green, 
blue.)

I explored the archives for embedding beads, but only found reference to a 
glass bead that is placed in the wax inside the block, but not subsequently cut.

PS: we are still troubleshooting our dyes (dilutions, brand, etc.), but if 
anyone has a brand of dye that they like and have no issues with, I'd be 
thrilled to get the information as well!

Good one Jay! I have had issue with a couple reference labs...

Yes... I had a Caris Life Science rep show up in blue scrubs (just like our 
facility). He misleads me into believing he was with the oncology group.  So I 
took time out of day to speak to him (thinking he was a Queen of the Valley 
employee, nurse or tech).
You can imagine how very annoyed I was to find out that he just dressed up like 
this because it gives him a better chance to wedge through the door of a lab.

Next is Genoptix... long story short: Genoptix faxed a request for blocks, 
reports, and slides so that they could perform a test for an oncologist... I 
have never heard of these people, never seen a rep, nor do I even have a CLIA 
certificate on file, and why do they want 'everything on file Seriously?

So I call them.  Come to find out, our current lab of choice does the same 
test.  Not to mention the courier was due to arrive in 45minutes to pick up our 
send outs.

I spoke to the customer service person at Genoptix... she was going to fax me a 
CLIA... so I decided to check with my pathologists.  The block went to our 
longstanding reference lab and the case was done quickly.  A few days later, I 
get a few inappropriate calls from Genoptix. They were very angry that they 
lost business because I did not send them the case. They also made sure to tell 
the oncologists that the pathology lab refused to send the block...

They went on further to suggest that I was sending the block to our current 
ref. lab because of some kick backs

Let me assure everyone on Histonet... KICK BACKS ARE ILLEGAL!   That being 
said, every reference lab we have ever used does not participate in 
kickbacks...  Genoptix though, that's another story. Pitting an oncology group 
against the hospital's pathology lab is a very poor way of getting the 
business. Those tactics don't go over well with any of us!

7:20 A.M. to try to refute one of the claims.  0720 isn't very early for a

We recently acquired the Bond III. It was a tough decision, but space was the 
main reason we went with Leica. The Ventana sales team that visited our lab was

I totally agree and we have an Ultra, 2 XTs and 3 Benchmarks.  They are good 
systems that need to get cheaper and more flexible.  I am demoing the Bond III 
and love the ease of use, flexibility and cost savings, most.  Maria is right 
it is about 40% less to run than a Ventana.

I have the Ventana Benchmark XT...love it..BUT Leica is LESS EXPENSIVE!!!



Reasons to buy Leica Bond:



1.Does IHC and ISH (yes- so does the Ventana)

2.   Continuous feed (Ventana does NOT offer this)

3.   Space saver, much smaller footprint than Benchmark

4.   No wasted antibodies...

5.   Not forced to buy EXPENSIVE prep kits either

You don't want to call it corrective action, as that indicates 'negligence' or 
'failure to follow direction' on the part of the tech... Unless that is the 
case and you are looking to go after an employee..

Instead you need to call it Performance Improvement... This notes a problem, 
reason for the problem, and a solution to the problem... followed up by a time 
line where you follow the change in Policy and Procedure.

It shows a problem that occurred where patient safety was compromised and a 
solution to the problem to avoid any potential future.

You document this... Say you change the procedure today... well, keep track of 
the IHCs for the next 2 months...6 months (what ever timeline you choose)... 
then write up the conclusion to your findings.

Here's and example: The change to preparing IHC slides per Policy and 
Procedure #2001 on May 1, 2010 and studied for 4 months,  we found no more 
issues regarding false positives.

There has been a lot of confusion about this. From my understanding the
real definition of who can gross falls under the High Complexity testing
category. This would be different for different states I think as well.
Let me give an example.

In the State of Florida pervious technicians may have been trained to
Gross large specimens( or biopsies ). With this new law a technician in
the state of Florida would not be able to continue to gross because in
actuality they never held a license that allowed them to perform High
Complexity testing. So they are not grandfathered in because of that
clause. Technologist would, because they have always been able to preform
High Complexity testing.

I also think that which ever laws your state goes by for people who can
perform High Complexity testing is your answer.

changing, and different certifying organizations have different
requirements. If somebody can spell out in detail who it is that's
requiring what, I'd appreciate it.

HOW TO CUT NAILS (or other difficult tissue) SO THAT THEY STAY on slide!

(1) Cut the slides and place on Plus(+) slide
(2) shake off excess water
(3) place slide immediately in plastic Coplin jar with 1-3 drops of formalin at 
bottom.
(4) place in hot oven for 5 mins (50-60 degrees)
(5) remove from Coplin jar under a hood as formalin drops are now carcinogenic 
vapour
(6) place slides in rack in oven so that the formalin moisture dries...
(7) slide is ready for use

Meditech is an awful program for Pathology. I loved using Pathologix (the 
Software company... not the Reference lab that is stealing histology jobs from 
hospitals).  Even Co-path is acceptable.

Meditech does not lend itself to Pathology as it does to clinical lab. It is 
cumbersome and not user friendly... no matter how well you are at using 
computers...

and as we all know, Pa's and Pathologists like having a requisition to make 
notes on  and refer back to.  We luckily are still on downtime forms and we 
will not be moving into using Meditech for Doctors' orders.

Just my humble opinion!

Hi All,

The problem is that when you receive it and then release it to the patient, you 
set your self up for a lawsuit.  Have you introduced some biological hazard to 
it by placing it on your grossing table? How long did it sit in your 
counter...fridge?

Some people bury it, but some people also eat it. Yes, sounds gross, but that 
does happen. Not sure which culture doe it...but I have heard of it.

For the same reason restaurants are governed by FDA, I would imagine the 
potential for a lawsuits if the person who may ingest the now contaminated 
placenta gets sick. Just a thought.

If you are going to do this, then let patient take it home. Don't even do a 
gross on it.

[Histonet] Leica Tissue Processor- Issue with Eosin in alcohol

Hi All,

Have any of you had FAILURE of valves or downtime due to over use of Eosin in 
the Leica Asp 3000?

I used to use 1-2 mls, never had a problem.  The Histotech that is no longer 
here (do the math) trained my new person to literally pour in the eosin. So 
possibly 10-30mls are going into each bottle.  Is that enough to 'make the 
valves stick'?

Please advise if you have had or heard of this problem.

I heard a few bad things about it...(not that I have ever used it)... But when 
you have tissues that had prions (that nothing but heat kills), its not a good 
idea to crush the slides. They need to get melted or what ever is done to 
sterilize them first... anyone else know about this?

I was a DAKO fan for years... But after using Ventana, I completely changed my 
mind!

Sure you pay a little more, but honestly the amount is negligible!  In  fact, 
after pricing out Dako versus Ventana, in the end the was really no difference. 
Plus the quality of the Ventana is exceptional. No one can touch their 
antibodies...

If they don't have the antibody, Cell Marque does. (Cell Marque even puts the 
antibody into a Ventana style dispenser so that you register it and run!!!) Who 
else does that? No one. Trust me, I have checked.

OMG... now I am sounding like and ad... didn't mean too!  I think Ventana is 
the best and you can't beat their customer service dept.. Dako falls short 
on tech hotline help Ventana will even try to help you, even if you have a 
Dako!  I am just saying.

Ventana reagents eem to be a little pricey, but if you want a system that you 
can load and walk away from, it is the way to go.  DAKO is an easy enough to 
use instrument and is an open system, but they are going to a price per slide 
kind of thing, if I remmeber correctly.

 Biocare, as is is an open system and a continuous load instrument.  And it 
mixes the chromagen online

I was a DAKO fan for years... But after using Ventana, I completely changed my 
mind!

Sure you pay a little more, but honestly the amount is negligible!  In fact, 
after pricing out Dako versus Ventana, in the end the was really no difference. 
Plus the quality of the Ventana is exceptional. No one can touch their 
antibodies...

If they don't have the antibody, Cell Marque does. (Cell Marque even puts the 
antibody into a Ventana style dispenser so that you register it and run!!!) Who 
else does that? No one. Trust me, I have checked.

OMG... now I am sounding like and ad... didn't mean too!  I think Ventana is 
the best and you can't beat their customer service dept.. Dako falls short 
on tech hotline help Ventana will even try to help you, even if you have a 
Dako!  I am just saying.

Ventana reagents eem to be a little pricey, but if you want a system that you 
can load and walk away from, it is the way to go.  DAKO is an easy enough to 
use instrument and is an open system, but they are going to a price per slide 
kind of thing, if I remmeber correctly.



I prefer the IP from Biocare, as is is an open system and a continuous load 
instrument.  And it mixes the chromagen online

[Histonet] New Grossing station for sale

Well, I was given the green light:

It's a Shannon Grosslab Senior Workstation.  It was delivered September 
2004...stored ever since due to the fact it was too big to instal in the small 
space. (Someone doesn't know how to measure!)

The item purchase price was $15,484.86 and has a current book value of 
$8,000.49 you know, depreciation!

It has never been used. In fact, in storage with the plastic wrap still on.

Let me know if you are interested. We are trying to get rid of it since we had 
to buy a smaller one to better fit our lab.

[Histonet] Trouble making a good Cell Block

Hi All,

Can anyone give me a good protocol for making a cell block from a non-gyn 
fluid?  (abdominal fluid, bronchial wash, pleural fluid, etc)

First of all, we have switched to formalin. Yes! Formalin!! So as you would 
expect, no button forms.

The reason? The pathologists believe the formalin is better than alcohol (95%) 
especially when you expect to run IHCs on the cell blocks.

We are NOT using Copath however...
I think the only time you have to separate Cytology from Histology is when you 
are dealing with GYNs. Otherwise, you can and should use the S09 (or what ever 
you give Pathology Surgicals). Why would anyone want to have two separate 
reports on one specimen?

Are there any sites that are using CoPath with one number wheel for
accessioning Cytology and Histology cases together?

Right now, we have two separate departments with Cytology ordering under
C09- and Histology ordering under S09- and we have have totally
separate specimen classes.

We are wondering if it's possible to use a single number wheel, using a
combined number such as P09- (for Pathology), but still have each
department order their own tests.  This way, for example, when Cytology
enters a FNA specimen in the morning and we (Histology) get a biopsy on
the same patient a few hours later, when we enter the patient we are
notified by the CoPath system that specimens were already assigned to
that patient and we can use the same accession number.  When the case is
complete, everything is in one tray and can be signed out all together.

One of our docs wants to have everything from Histology and Cytology on
one report for the patient.  Is this possible?  Can Cytology still
result the case if we have one accession number for everything?

Any help or ideas on this issue would be greatly appreciated.

I wouldn't say that histology is a dead end job, I mean, I jumped to a PA
and I know that I couldn't have passed that exam without my histology
experience. The idea of adding an HTL category was initially a good one.
Trying to bring us up to the MT and CT levels, but honestly, I think it fell
short. Even after I received my BS, people asked me if I was going for my
HTL. I told them no because I saw no benefit from it. At that time, I felt
that if I couldn't get hired as a supervisor with a BS and 20 years
experience, with 12 as a supervisor, I wasn't going to get the job any way.
I'm not degrading HTLs by a long shot. I'm just saying for me, it wasn't
worth it. After I retired from the Air Force, I was directly hired as a
supervisor without the HTL. Just my three cents (inflation you know)

I couldn't agree with you more. I'm in the same position as you. In the
Boston, Mass. area people are taken right off the street and work for a year
as a lab assistant then promoted to a tech in training. Most have a hs
diploma, no ambition and expect good pay for bad work and poor work ethic. I
have been in the histology field for 20 years and don't consider it a
profession or a career, just a job.

I am a rarity. I am an HTL with a Bachelors Degree. I got my HTL in the
early 90s and I guess I was misguided because I thought it would open more
doors for me than just an HT. I was sadly mistaken. After I passed my test
I waited 9 months for a raise and promotion (which was just a greater title)
and when I got my raise so did 2 other employees that didn't even have or
try for their certification. I spent many nights and weekends studying and
doing my stains for the test. I am proud of my accomplishments. It is a
shame that our industry does not reconize the difference between HT and
HTL. A few years back I was working as a traveling histotech and when I
tried to get a permanent position no one wanted to hire me because I was
over qualified by having over 15 years experience and a HTL certification.
I worked hard to no avail. The histology world doesn't look for well
qualified workers they look for cheap labor. I have heard more than one
pathologist state that a monkey can do our job. I have also worked in a
lab where they would hire someone with a GED to cut slides. A career in
histology is for the most part a dead end and there is no future. As long
as our industry doesn't respect education and experience there will be less
and less histotechs and the quality of the slides will suffer which in turn
will bring down patient care.

You would want to hire an HTL if you were going to groom them to be a manager.  
Wouldn't you want to have a person with a BS at minimum to run the department???

There is something to be said about an educated employee.  Also, most hospitals 
'require' a BS at minimum for any supervisor or management position. That's how 
you stay competitive.

Experience is great, but getting a degree says a lot about a person, especially 
in today's economy!

Be careful... Nexus waste DOES contain silver waste if you run a GMS, 
Steiner, etc and therefore, as a heavy metal, never go down any drain... no 
matter what state you live in. I am almost sure it's a federal thing...  waste 
and waterways or something like that.

Our IHC waste is hauled away like our Alcohol/Xylene sub. The town we are in
would not allow the IHC waste down the sink, but they do allow us to throw
the NexES sp. Stainer waste down the sink. Maryland is the state.

Please for those of you using a Ventana IHC instrument and/or special stains
instrument, I am interested in what you do to dispose of your toxic waste.
I know each municipality is different so let me know what you do and where
you live.  Ventana does not separate toxic from non toxic waste so I imagine
there is a lot of toxic waste to deal with and the liquid coverslip alone is
something that I would think should not go down the drain??

Why would anyone open a new lab, give cytology a hood yet have a complete 
disregard for histology area?

I am so glad I work for Queen of the Valley Medical Center... they actually do 
care about ALL of there employees, no matter what department they work in.

To answer your question about fumes hoods and special stains:

I am sure there are many histotechs that are subjected to fumes from the 
special stains they do. This is because no one thinks about the consequences of 
us breathing heated GMS solution (heavy metal in vapor form) or phenol from the 
heated AFB solution you just used...or the formaldehyde (carcinogenic fumes) 
from the retic stain... Should I go on?

This may be to norm out there, but its very very wrong and asking us histotechs 
to work under unsafe conditions!

You must do you specials under a hood for your safety and/or be given proper 
PPE (personal protective equipment)!

Print this e-mail and have your boss, medical director, (or whoever else thinks 
it's ok for an employee to be subjected to potentially harmful fumes) call me.  
I will be glad to talk to them about the danger of working in an unsafe 
condition.

Have them look at the following OSHA Guidelines:

Section III Chapter 3 Health Hazards/ Ventilation

Section VI Chapter 1 Healthcare Facilities

Section VIII Chapter 2 Personal Protective Equipment

This is not much help, but maybe it will open someone's eyes to the legitimate 
concern you bring up.

The hospital where I work just opened a new lab. The histology department is 
now combined with cytology and is made up of me, a second histotech, a cytology 
prep tech, and our cytotech supervisor. We didn't have a hood in the old lab, 
so we did our manual special stains on a workbench with weak, overhanging vents 
that looked like lamps. The one hood in our new department completely belongs 
to the cytology prep tech. She has her centrifuge and a lot of other equipment 
in it, and my supervisor does not want me or the other histotech to share it. 
Some of our workbenches have small vents built into the walls, but when I do my 
specials there, I still smell chemicals. Do most histotechs today use hoods to 
do manual special stains or am I being too wishful?

We track from time of receipt.

One way you could track from time of order is if the specimen is ordered
electronically. We have that set up for some of our units - a
requisition is ordered with time of order, etc. But we couldn't get it
working in the OR - not enough people and printers at ready access..

CAP utilizes the term intraoperative consultation to describe the
utilization of frozen (cryo) sections to provide a rapid diagnosis back
to a surgeon in the operating room.

The CAP checklist requires a turnaround time of 20 minutes for single
specimen submitted for intraoperative consultation. My understanding is
that the turnaround time is measured from the time the sample is
received in the laboratory until the time the report is issued to the
surgeon.

Is anyone tracking or measuring turnaround time from the time the
consult is ordered in/by the Operating Room until the time the result
is issued?
If so, would you share how you are able to determine the time the test
was ordered  and to what extent you have elicited the cooperation of
Operating Room personnel.

We receive many complex surgical cases and our intraoperative consults
frequently consist of multiple surgical samples from the same patient
arriving in the lab at the same time. Our head and neck cases, for
example, consist of 6-8 biopsies that are sent to pathology at the same
time. In this example, we have no knowledge of which biopsies was
excised first or last and because the surgeon chooses to allow multiple
samples to accumulate before sending them all off to the lab, it's clear
that the true pre-analytical time will not be the same for each
sample.

If you are tracking turnaround from the time of order to the time of
result reporting, how are you determining what is an acceptable
turnaround time? CAP's standard is the only national standard I am aware
of for frozen section turnaround times.

We are currently working with architects on designing the new lab we are 
building. This particular item is one of the projects we are working on. I will 
let you know how we decide to have connectivity to OR.

Remember, that is you spend just a little more, you can us a CSTV close circuit 
tv.  This way OR can see how the pathologist ink  orient the specimen! I will 
keep you posted. Its not as expensive as you may think.

We have a very old speakerphone system that we use to call from the 
pathologist's office in to our operating rooms to discuss frozen section 
diagnoses with the surgeons. Both sides are yelling and straining to hear and 
we're concerned about the risk for miscommunication. We can't go into the OR 
directly, since Pathology and the OR are too far apart physically.

We're looking for some sort of telephone-based communications system, perhaps 
with a speaker and microphone that can be mounted near the surgeon, so that 
when we call into the OR with the frozen section diagnosis, it can be switched 
to speaker and the call can be continued with direct and audible communication 
between the pathologist on the phone in his/her office and the surgical team at 
the head of the operating table.

Does anyone out there have a system like this?

Do you know of a commercial vendor who makes something that would work?

We tried an expensive Polycom system meant for conference calls, but its 360 
degree microphones picked up too much background noise in the OR.

It seems like it should be a simple Radio Shack project, but we're all 
techno-challenged.

Any help would be appreciated. Thanks.

I had a group here who were having trouble with their cryo-sections
sticking
 to the slides. Their samples were rat brains. He found that the Gold Super
 Frost Plus worked well with in-situ but not for immuno. At least that is
 the
 comment he wrote down and put on our bulletin board in the lab. Most of
 the
 people in the lab tend to use the Super Frost Plus (not the Gold) and they
 seem pretty happy with them.
