From my limited understanding, it is an 
itching condition of pregnancy.  It stands for something like pruritic 
urticarial papules and plagues.  I knew I had heard of it and I've looked 
it up in one of my many books - found something about it in Deborah Cooper's 
book "Your Baby, Your Way".  
 
In it she recommends:
- applying 100% jojoba oil
- applying chickweed or nettle 
ointment
 dandelion beverages instead of 
coffee
- a glass of beetroot, celery and carrot juice per 
day

Dear Wendy - I began an assignment with this in the 
opening paragraph.
 
Fuchs (1983 in Nichols and Humenick, p1, 1988), tells us that 
of all life choices, none are more important to society, none has more far 
reaching consequences, none represents a more complete blending of social, 
biological, and emotional forces, than bringing another life into the world. 
Similarly, Maynard (?) draws our attention to the fact that while giving birth 
can be a moment of pure ecstasy and the beginning of a greatly enhanced sense of 
self-worth, it can also be a shock from which a woman never recovers (in Nichols 
and Humenick, p 52, 1988). 
 
The reference is Nichiols and Humenick 
"Childbirth Education: Practice, Research and Theory" Saunders International 
1988. There is a second edition out now - but hope you find this 
useful.

well Cheryl, in my copy of Maggie Banks' "Breech Birth Woman Wise" I 
found a statement - " Where the baby is born to the umbiblicus, and 
therefore the head is in the pelvis, the baby will need to be born 
within 10 minutes". the reference for this is cited as Stevenson, J. 
(1993, Summer) More Thoughts on Breech. Midwifery Today. No. 26. pp24-25.

Hello All

Great to meet many wonderful midwives at the Homebirth Conference in Katoomba!  

In our continued lobbying efforts I really need to hear from regional midwives, we are looking at an electorate strategy for federal lobbying.  I have just browsed trough the ASIM directory but would like confirmed from midwives who was or is working where.  Particularly is anyone working in the Central West/ North West of NSW? (Dubbo particularly).

haha Im laughing  bcoz Ive just bought this book by Maggie Banks & I'm 
sending off references from it already !:))
(the 2nd today). anyway, Maggie talks about the 
Mauriceau-Smellie-Veit grip, being what you do to assist the birth of 
the baby with an extended head. What it basically does is apply pressure 
to the cheek bones & the parietal bones, which brings the baby's head 
over the perineum while protecting it from sudden decompression. (Not 
what you asked about but thought it was worth including) She also 
follows with a bit about moulding of the head, & I quote - "It is often 
cited that the baby's head will not have the time needed to mould or 
shape as it comes through the pelvic bones", then her own experience 
which refutes this - "I transferred Kitty to hospital following 20 hours 
of labour with her breech baby. She decided to have a c/s as her labour 
was slow. The baby's position had been anterior during labour. Anna's 
head was not the usual round shape of a breech baby born by c/s. The 
baby's head, which had been in the fundus of the uterus, was moulded to 
be oval in shape with the overriding of the skull bones. It had been 
moulded by the contractions that she had experinced prior to the operation".

I can't remember exactly what the initials stand for,but it's essentially
severe itching in pregnancy - the U is for Urticaria it think.
The thing that bugs me here is that they give benadryl for it postpartum
which I don't like becaus it's an antihistamine and can affect early
lactation.
Also, anyone with severe itching in pregnancy should be also assess for
Intrahepatic Cholestasis of pregnancy because of the possible poor fetal
outcomes

Yes, I agree with the previous definitions (but can never remember them all) 
I think the essential thing to remember is that the pruritis/severe itching 
can also be a symptom of choleostasis and so liver involvement should be 
ruled out. Once liver involvement is ruled out it appears that PUPPS is 
essentially a benign condition with extremely irritating symptoms. If 
antihistamines work, then the woman is lucky and she should get what relief 
she can, I personally haven't seen mum's get much releif until the the 
condition itself passes.  marilyn

Interesting quote Jenni. It is also interesting that the baby seems to me to 
have been in an extended position. The "ideal" position for a breech baby is 
fully flexed , which would (it seems to me) prevent the head from receiving 
moulding from the contracting fundus. As I said previously I have only been 
at 3 vaginal breech births and none of the babies had moulded heads, a very 
small number to draw any conclusions from (I have also been at vertex 
presentations with no apparent moulding). Marilyn

My apologies to the list - PUPPs stands for Pruritic Urticarial Pustules and 
Papules of pregnancy.  From what I understand it is a skin condition only 
and not something more sinister (my friend has had liver function and 
biochemistry to exclude cholestasis and all results have come back normal).
It is however causing her unbearable itching and stress - lack of sleep 
compounding the discomfort and wearing her down.  She is 32 weeks but 
already asking for induction (the itching usually subsides once the baby is 
born).  I have a feeling that for her, it is related to her worries about 
being at home with two small children - feeling isolated and with no family 
support and few friends.  We have talked about hypnosis as a way of managing 
the itching and to help her get some sleep and she is keen to give it a go 
but neither of us know where to start looking.  She is in Melbourne.
I'm really hoping someone on the list can help.

There IS moulding in some breech presentations

Hi all,
My second baby was born breech and she DEFINITELY has a moulded head -
slightly crooked as well. Will post a picture if I can a little later.
She arrived in a 7 minute pushing stage - 3 contractions - I held the head
in probably a little long as there is no final pushing urge with a breech
once the head is through the pelvis and actually sitting in the vagina. It's
very much up to the mum to 'let' the baby out by opening the pelvic floor
muscles. I have helped a number of women with breeches, and all have found
the right way of letting the baby's head out except one where the doctor was
pulling ...
So my breech baby has just left home and is in her second week of medicine!!

And one doctor told me I was lucky she wasn't brain damaged!!

Lucky? Anyone who doesn't understand the enormous emotional journey it is to
decide to birth a breech baby vaginally shouldn't be involved with birth!!

My choices - breech vaginal at home or c/section - 18 years ago - I'd have
to say the choices have not improved, but rather, fewer homebirth
practitioners are available and fewer again who willingly attend breech
births.

Back at my computer/Port Macquarie

I have been having trouble with my computer and after three weeks without it
have just bought a new one which once I learn to drive it will be OK. Just
when you get really confident with how everything runs they die and you have
to upgrade to a new program that is nothing like the original. Oh the joys
of it.

A reminder to anyone keeping their client files or anything else on computer
to have a good back up system and use it because trying to retrieve them
from a damaged hard disk or losing them is no picnic.

I have been approached by a woman who is moving to Port Macquarie and is
looking for a midwife. She is planning a hospital birth at this stage but
would is interested in having her own midwife with her. Any takers as she is
moving up there in the next week or so

I'm so disappointed to have missed the HBA conference but I have beautiful
birthing to attend on Saturday night. Spontaneous onset of labour at 42
weeks in my first repeat client. My next excitement is my first client who
is planning a home birth is due this friday and I'm not sure who is more
excited, me or her. I'll let you know how we go.

I recently received a flyer from Cryosite cord blood service with my ACMI 
News/journal.  The company is subtly using midwives to market their 
service, the text reinforcing in our minds that the blood should be stored for 
the child's future use.  Maybe we should be reinforcing that the blood 
belongs to the baby at birth and was put there for a really good 
reason...strengthening the immune system.  Maybe if the baby receives its 
birthrite in the minutes following birth, it will not need it later.

Something like Puerperal Urticaria of Pregnancy...
In other words a symptom of Obstetric Cholestasis, I found a website that
would be useful, next time I'm at work! I'll post it if no-one else has.
