There is no such document on CCO. For the lab exam, you have to know
beforehand how MGCP, H323 and SIP work.
You dont have to know every possible debug output, but you must know what
message comes after what message and what most messages mean.

You do not need to add the GW to CUCM; otherwise this will not be scalable
at all. I had the same confusion but then I got it to work without adding my
GW (CME) to CUCM.

I think every CCIE-to-be should take this approach and analyze the protocol
himself (MGCP/SIP/H.323), as this is the only way to understand what's
really happening behind the scenes.
Plus in the actual lab, there is no time to read documents.
