OK I finally got qmail-remote to run under truss from the command line. Here

Have you tried looking at /var/qmail/alias/Maildir ?
qmail won't deliver to the system-user root, to prevent possible exploits.
This can be solved with a .qmail-root file containing
&[other username to forward to]
