Moving / deleting the Preview plist (preferences) file sometimes works.
But if you want the more reliable fix (which doesn't reset your
toolbar preferences each time), locate this file:
PDFIndex4.sk
Located at:
/Users/username/Library/Application Support/Preview
and

preference file.
Quit Preview, move the com.apple.preview.plist file out of
/Users/username/Library/Preferences/ to the Desktop, relaunch Preview, and open
an image file.
