There is still a lot to be improved on in the services out there though.

Personally, I friggin hate receiving these SMSes, appointments go into my
calendar immediately, I don't need no stinkin reminders. But there's no way
to opt out. I'd love to be able to just reply with "NEVER AGAIN" and the
system takes care of it. I've spoken to a couple of service providers (in
this scenario - people I'm making appointments with) about it and they've
said it's automatic, we can't change it yada yada.

The other thing I'd love is to be able to reply and get an appointment
changed automatically without having to leave sms and call the service
provider. Some of them send an SMS where you have to answer Y/N to
'confirm' the appointment. Surely that can be extended to an automatic SMS
appointment making system. "Change time". "New time: 4.30pm Y/N?" "Y" New
time set!
