All good questions...

I'm slightly embarrassed to mention this...however I've just realized that the 
ITSP that I thought I've been routing thru, is actually not the one I've been 
routing thru.  On that note, I'm going to make some configuration changes and 
re-test.  I've inadvertantly been using callwithus - I had meant to be using 
sipstation (bandwidth.com) for this...

Once I'm actually sure which SIP trunk I'm using, I will report back

Calls failing after about 30 seconds (32, to be exact) are usually indicative 
of misrouted 200 OKs or ACKs.  In an effort to try to isolate the VPN as a 
contributor to this problem, have you tried making an ITSP call from an 
identically configured x-lite that would be located in the courthouse?  Also, 
what ITSP are you using?   Are local users (the ones at the courthouse) able to 
make ITSP calls without issues?
