If you are using an older Polycom phone, like the 301, 501, 601 or even older, open the 000000000000.sip file and look at the file they are looking for, as it is an older version of the firmware. You will see a reference to the model of phone, and the exact LD file it is requiring, and it will be different than the other phones. If your phone is not one of the referenced models in that file, then the device files you load from the GUI will work for you.
Create a file called [mac]-boot.log and a [mac]-app.log in that tftp folder, and reboot your phone. The log file will show you what file it is looking to find, and provide some clues to your issue. After creating the files, change their properties to sipxchange and 0666.
