Good call Jeff, I've been working with the poster off-list sending a bunch of emails with the test-point URL in it and this appears to have been the problem.
I just sent off one more test, but the previous one appears to have been picked up correctly, so I think it's been resolved.
(Note: the actual problem was Amavis's sa_local_tests_only setting instead of -L, however it's conceptually the same problem).
