How about something like a U. V. International 11" x 4" ductile iron threaded 
reducing companion flange (DPF0411) with a 0'-4" piece of schedule 40 
galvanized pipe thread x cutgroove?  Takeout would be about 5".  A question 
though is whether or not there is a meter device on your BFP that may give 
erroneous readings due to the turbulent flow created upstream of this fitting 
if located immediately upstream?

I am pretty sure that Vic makes a whole line of AWWA approved products, they 
use the same number but add a different letter to the end, 50-C is a concentric 
reducer I think.

I've done it for years as well, but the National Plumbing Code prohibits the 
use of any pipe or fitting that is not AWWA approved upstream of the backflow, 
and the AHJ is enforcing it. Neither black nor galvanized steel pipe is AWWA 
approved. We have had to buy cement lined 90° and tee's in the past to comply.

Use a 6" steel blind flange and have your shop cut a hole and weld a

 

4" PEx Gr piece of pipe in it to the length you need, this has been

 

done for years.
