That's just screwy. Do you also have an Ultratek/66 installed in the same machine? I could see the machine maybe somehow losing track of which slot which firmware belongs to.
You may wish to reset your NVRAM if you have not already done so. It is also possible that if you replaced an Ultratek/66 with the Acard that the Open Firmware Device Tree still has info for the Ultratek/66 and has not updated it properly for the Acard.
The Acard should work fine in the lower slots. At least, I have one in my lower slots and it works trouble-free for me. That's one of the big reasons I ditched my VST card and went to the Acard.
On the other hand, I'm still using OS 9.1, so if your issues are OSX specific, my experience is not relevant.
