I have seen this before with bad/conflicting drivers under XP. The usb-serial 
adaptor is not 'ejecting' correctly or is being held active by another device. 
Clean your USB system as follows:

- Download USBDeview (http://nirsoft.net/utils/usb_devices_view.html)
- Show all device types under options
- Select all devices in list
- Uninstall all selected devices
- Reboot

Make sure you have the very latest driver for your device. Most are based on 
Prolific hardware. This driver works for me:
