Something's wrong, probably with your _from header. Or maybe Cox is blocking bounces, which could explain why mail sent via mutt works, but mail with a <> as the envelope sender doesn't. Try setting HOSTNAME in your tmda config to whatever value you're using for your SMTP server in your exim config.
