The setup I had was for each individual machine. I can't remember exactly
how I did it anymore, but it I think I used either bridging or connection
sharing in XP. (I'm working on recreating it so that I can document it like
I should've the first time.) This way no other machines needed to be on and
I didn't need a certain wireless router that I could setup rules as you did.

Having everything self contained in one appliance would be even better

I agree that a nice simple appliance would be better. The reason I developed
the solution I did was that it was free. I developed it for my dad, who is a
bishop of a student ward. He wanted to be able to give something to the
members of his ward that they could use to filter web traffic. Most students
don't have too much money floating around so I opted for that route. Alas, I
never got it into a form that he could use since I didn't have enough time
to take it further.
