Ok, I do this now but I still wish it would tell me where I am when I hit
that app switcher! I guess I like more rather than less verbosity. What is
nice is that if I simply hit the home button once, it will bring me back to
the home page where I might want to open something else without closing
settings or whatever app I was just in, so that is easy!

I think I see what you're saying. It isn't that the focus is on the wrong
app, it's that VoiceOver is silent, not speaking the app at all. Simply
touch the screen to focus; you'll have to do that anyway, to close it, or to
start swiping to other apps, so it's really nothing extra.
