What probably happened when VoiceOver stopped working is that your phone interpreted whatever you did as a three-finger double tap, which turns speech off. Just three-finger double tap it, on purpose this time, and it should come back on. I do wish they would let turning speech off be the three-finger triple tap, which now toggles screen curtain on and off, and let the three-finger double tap toggle screen curtain on and off. Having speech turn off, and not knowing why, caused a few don't know what to do moments when I first got my phone.

For me the rotor bug is not just the 25 percent limit, but also that if you move it from five percent to ten percent and keep going the same way sometimes you go back to 5 percent instead of on to 15 percent.
