Andrew:

1. in addition to console.log what does system.log say at the time of the 
failure?

2. confirm that -

a. the host name that you're passing as -WOHost to wotaskd, JavaMonitor,
b. the host name of the server
c. ServerName in httpd.conf
d. the host you added in JavaMonitor to run your apps

are all exactly the same.
