By any chance did the case use screw in stand off spacers ?

Did you check for any in the wrong place ? 

I had a machine that, after varying amounts of time, would simply die !  
It turned out that a stand off had been screwed into a wrong hole, just 
under the right hand edge of where the CPU socket was.  One of the four 
Used to mount the CPU fan on some machines. The board got heated up 
enough to cause the stand off to short out the 5 volt line.  It did no 
damage, other than to kill the PSU. 

I have also had a shorting problem with cases that use pressed domes in 
the mainboard mounting plate !!  But in every instance it kills the PSU 
stone dead the instant the power button is pressed !  Lift the 
mainboard and everything is Ok.  Only happens with ASRock mainboards as 
far as I am aware.
