Rory's points are also equally valid if you turn the platform around the other way - there's a number of ISPs that have *zero* Microsoft infrastructure on the back end.
If you have a great depth and breadth of in-house Linux/BSD sysadmin knowledge it makes sense to go with a custom in house built IMAP (TLS1.2) solution for email access. If I had something broken and Microsoft related sitting in front of me it would be a large learning curve to figure out what to do, and how not to fuck it up in the process. The last time I had admin rights on a system that ran Exchange it was v5.5 and ran on NT4. Just as some ISPs would have to pay a third party consultant for Linux stuff, I'd have to pay for Microsoft... I am not about to spend the time/money/effort to become fluent in all the myriad licensing issues and systems deployment requirements to built a reliable five nines Exchange server on a Windows platform.
