add the clamav user to the amavis group.
# grep clamav /etc/group
...
vscan:*:123:clamav
(here, vscan is the amavis group)

Look at the logs of amavisd-new when it starts. it should say something 
like this:
... Using primary internal av scanner code for ClamAV-clamd
... Found secondary av scanner ClamAV-clamscan at ...
