Have you tried resetting the user's password to something simple (i.e all
lowercase and not symbols, if you're AD allows that) and then have them try
logging in to Remedy?

I'm wondering if something in the case of the password or perhaps in a
symbol could be causing this.

The other day when I was watching the user login, they did not mistype
their password - which is part of what makes this so strange! :)

This may already have been mentioned, so forgive me if I'm being
redundant.  The old 5.1.2 client (unpatched) did the same very thing
you're describing...but only if the user mistyped his/her password.
With this issue, if the user mistyped his password just *once*, boom,
his account was locked out.

I hope that BMC did not reintroduce this bug (or one that's similar)
when they released the newer client.

We had this problem back when we were on ARS 6.3.  At that time we were
just cross referencing blank password and did not have AREA LDAP
configured.  Once we configured AREA LDAP, the issue went away.
