The old 5.1.2 client (unpatched) did the same very thing you're describing...but only if the user mistyped his/her password. With this issue, if the user mistyped his password just *once*, boom, his account was locked out. At that time we were just cross referencing blank password and did not have AREA LDAP configured. Once we configured AREA LDAP, the issue went away.
