Hello Raj, What Patch level you are on for SRM 7.6.04, We have seen such issue with unpatch version of 7.6.04 and all of them got corrected in 7.6.04 sp4. Also when you adding an attachment, do you select the internal or Public? when you select public (radio button), it will be display within activity log.
