Starting in chapter 3, images are missing in your PDF. Figs 46, 50, 52,
53, 55, 56... and many more throughout the book. They appear to be the
same ones that are missing in the PDF I created from the same files on
my machine, but I have not checked all the way to the end of the book.

We may have the start of a bug report here, with a bit more testing to
see if it's reproducible on other operating systems.

Later today I will look at the incorrect x-refs and see if they are the
same in both PDFs. Now I wish I had kept a complete list of the ones I
fixed yesterday, to see if any of them have returned.

I just accepted all the default export settings, so if you need something else, let me know. Export takes about 10 minutes, which is about the same time the master needs to load, but the export has a nice progress bar.
