Starting in chapter 3, images are missing in your PDF. Figs 46, 50, 52,
53, 55, 56... and many more throughout the book. They appear to be the
same ones that are missing in the PDF I created from the same files on
my machine, but I have not checked all the way to the end of the book.
We may have the start of a bug report here, with a bit more testing to
see if it's reproducible on other operating systems.
Later today I will look at the incorrect x-refs and see if they are the
same in both PDFs. Now I wish I had kept a complete list of the ones I
fixed yesterday, to see if any of them have returned.

I do not see image 46 from the Master document view. What I see in the
Master document appears to be what I see in the generated PDF. I do see
image 47, but, others are missing as well. I did not test further to see
what other images are missing.
Note that I was testing the latest 4.4 dev build for my tests because it
contains a fix for my previously most hated bug that caused documents
with many style changes to crash OOo....
