Many of the problems around this technology is the complexity of the vocabulary desired. The less commands you want to recognize, the better the accuracy. From a practical standpoint I always had problems with Dragon while taking dictation or recognizing commands and controlling the computer because of family interruptions or background noise. It can give you strange results and take longer due to undoing and editing! It was kind of cool, but very frustrating!

Recently, several mainstream devices have come closer to your ideals. There is the Ford in car electronics which uses voice control of the audio, temperature read email from your bluetooth phone, or send a text message through your phone, and so on for a hands free experience. It sounds pretty imprressive! It almost makes me want to drive. It is even in their low end Focus cars. Smart phones are surprisingly getting pretty good at this as well I hear. Google has many projects that allow voice input and recognition for search and translating voice mails to text emails on Android phones. Most of the heavy processing in these cases is performed on their servers.
