I've been through all of this only it was with a hospital, and my
insurance DID cover it, and they were late paying the bill and the
hospital sent collections after me....and after a long (3 years) drawn
out fight, the insurance paid what they were supposed to pay in the
first place, but because of the friggin (insert other derogatory terms
here) system and how screwed up it all came, I ended up being
responsible for all interest and legal fees associated with the
collection, even though the insurance was clearly in the wrong.

Throughout all of this, the lesson learned is keep track of EVERY
conversation, email, piece of paper, cancelled check, people's names, no
matter who it was, whether secretary or someone important, phone
numbers, dates of calls, etc.

Put it in diary format ... And this is the only ammo and evidence you
will have to defend your situation.

Another handy trick is to find a lawyer friend or even a para-legal, and
have them write an official letter and provide copied proof of the above
stuff.

This can sometimes help. OH! And the next really IMPORTANT step is to
write the insurance regulations company a really SCATHING letter
(preferably from said lawyer friend on their stationary) includding a
copy to aforementioned friggin insurance company ....

Lot of work, but may well be worth the payoff.
