A couple of advantages to having a form from off the top of my head:


1) People that use the local library or a friends computer, who has no
email address but would give a phone number for contact instead, can
then use the form.


2) I haven't tested recent versions of AOL, so I may be incorrect, but
back in AOL 5.0 life, clicking on an email address from a webpage, did
not work. You had to copy and paste, and I know MANY people who are copy
and paste challenged, so a form would help here.

Like someone else mentioned, keep the form simple and put explicit
directions at the top for the technophobes.
