This is not tracking changes. All you need to do is say that you
modified the document (title, year, new authors and publisher). You are
not required to state what you changed. It would look like this:
HISTORY
My Document - 2004-03-21 - Daniel Carrera - My Publisher
My Document - 2004-03-28 - Sophie Gautier - My Publisher
My Document - 2004-04-01 - Jean H. Weber - My Publisher
This requirement is only slightly annoying. It doesn't have any of the
problems I described for the PDL one.
PDL says identify instead of describing... But this not the topic of the
discussion here, that was your assertion that changes are never tracked
(and other licence like that for documentation exists, see the ACDL or
OPL too that are recognized as FSF free documentation licences).
And I completely subscribed to what Scott explained about protecting
users/authors/documents.
