That's because quoting efficiently, branching threads when required etc. are becoming a lost art unfortunately. Big +1 and I'd argue that documents are *not* a good way to make that happen. Git tickets for example work extremely well IMO for asynchronous collaboration on concepts, guidelines etc. But as I said whoever does the work decides which tool to use.
