When I've run into this, since media is cheap, I simply create a second file, one for print-out, one for playback. In the case you're discussing, I simply insert the extra bar, make it for the desired amount of time, make the rests accordingly, and save it as a MIDI file. In this manner, I can make one file look the way I want, and the other sound the way I want.
