I have seen installations send an object deck on a tape (another extreme was an object deck in punched cards!) the person who installed one product hid it in an undocumented CSI and it took some digging just to find the CSI as he didn't bother to document it, I had to guess the name. What a PITA.
Of course he was a short term consultant.
Since then I have suggested the consultants stay away from installs.

I've sometimes had to spend time figuring out what the correct CSI is.
SMP/E can be very nice, but their is still a role for documentation.
