Because it takes effort.

The  "end  user"  of  the  IP  address  has to get the "owner" of the IP
address  to  either create the PTR record in their tables or to delegate
the requisite in-addr.arpa range to them.

Some  providers  understand the need and all it takes is a phone call or
e-mail to the appropriate party.

Others   either   have   no  clue or insulate their knowledgeable people
until you jump thru hoops to get to them.

Either  way it could take persistent effort to get it done and until the
pain  of  not  having PTR records exceeds the pain of getting them setup
properly many admins just ignore the issue.
