it is most likely that the off screen model became unstable. often,
pressing ins+esc will fix this problem. if it happens again, see if you
can remember what you were doing just before you minimized all apps and
let us know about that.

ctrl+home should do this.

I'm not sure what you want to know. if you want an answer as to why it
is inconsistant, it is I guess that computers are like that especially
microsoft. if there is something I am missing with this question, Let

because the page is already in the cache and you are most likely
returning to the point in the page where you left off.
